The introduction of second-generation antipsychotic medications SGAs, see Box 1 ; was associated with a further increase, variously estimated at 10%50%10 Level III evidence, Box 2 ; . The contribution of improved screening for disorders of glucose metabolism is not known but this represents a potential artefact. A group of American organisations led by the American Diabetes Association recently published the outcomes of a consensus development conference on antipsychotic drugs, obesity and diabetes.13 The conference recognised that SGAs are associated with adverse effects, including obesity, diabetes and dyslipidaemia. It concluded that clozapine and olanzapine are associated with greater weight gain and a higher occurrence of diabetes and dyslipidaemia than risperidone and quetiapine, which appeared to have intermediate effects. Aripiprazole and ziprasidone not available in Australia ; did not appear to be associated with significant weight gain, diabetes or dyslipidaemia, although they had not been used as extensively as the other agents. The conference recommended baseline screening and ongoing monitoring for the development of significant weight gain, dyslipidaemia and diabetes. The American Food and Drug Administration FDA ; recently requested that all SGA manufacturers include a warning and additional information about a link between SGAs and hyperglycaemia and also recommended regular monitoring of blood glucose level. The FDA acknowledged that the relationship between SGAs and glucose abnormalities is complicated by an increased background risk of diabetes in people with schizophrenia and the growing incidence of diabetes in the general population.12 There have been case reports of apparent diabetic ketoacidosis occurring during treatment with SGAs. Schizophrenia is the most common severe mental disorder, affecting approximately 1% of the population. Studies of dietary calcium's effect on stone recurrence rates have led to major changes in nonpharmacologic manipulation. Epidemiologic evidence shows an inverse relation between dietary calcium intake and recurrence rates. This is probably best explained by calcium's inhibition of intestinal oxalate absorption. A randomized controlled trial RCT ; assigned men with hypercalciuria to follow either a diet low in calcium 400 mg ; and oxalate or a diet higher in calcium 1200 mg ; with restricted intake of oxalate, protein and salt.1 At 5 years, the latter group had a 51% lower rate of stone recurrence than those following a low-calcium diet. Although controversy persists and replication of these findings under other clinical circumstances would be desirable, low-calcium diets are not recommended and can exacerbate the well-documented association of hypercalciuria with low bone mineral density and increased fracture rates.2 Whether calcium supplements could have a similar effect in lowering stone recurrence rates has not been tested. While no studies involving clozapine Clozaril ; or ziprasiodne Geodon ; use in elderly populations have been completed, the FDA mandated that they, too, carry the new warning. The FDA noted that the 6 atypical antipsychotic agents fall into three different chemical classes, and the increased risk was seen with drugs in each of the 3 classes. Therefore, the FDA has concluded that the effect is probably related to the common pharmacologic effects of all atypical medications, including those that have not been systematically studied in the dementia population. The reviewed studies included 5, 106 patients, and several analyses demonstrated an approximately 1.6- to 1.7-fold increase in mortality in these combined studies. In these studies, which averaged 10 weeks in duration, the rate of death in drug-treated patients was about 4.5 percent, compared with a rate of 2.6 percent in the patients taking placebo. Examination of the specific causes of these deaths revealed that most were due to either heart-related events heart failure or sudden death ; or infections mostly respiratory, including pneumonia ; . In addition to the six antipsychotic agents, the olanzapine fluoxetine combination Symbyax ; will carry the new black-box warning. The agency also noted it may require the older, typical antipsychotic medications to carry similar warning language because the limited data available suggest a similar increase in mortality for these drugs. However, the review of data on the older drugs is ongoing. Clozapine, the oldest of the newer atypical antipsychotics, is associated with serious blood dyscrasias, including agranulocytosis, that have resulted in patient deaths. Clozapine also carries a black-box warning regarding cardiac toxicity leading to potentially fatal myocarditis. Both risperidone and olanzapine currently have warnings about potential increased risk of cerebrovascular events, including stroke and an increased risk of death from stroke. A similar warning was added to the label of aripiprazole at the end of last year. Ziprassidone was required at approval to have a warning regarding the drug's propensity to prolong the heart's QT interval and potentially to induce fatal cardiac arrhythmias. But the most studied adverse effect of the newer generation antipsychotics is their association with hyperglycemia, in some cases leading to ketoacidosis, coma, or death. A black-box warning was added to the labels of all six drugs in 2003 alerting prescribers and patients to the possibility of these drugs causing diabetes.

To treat these patients through ablative procedures, and recently, deep brain stimulation DBS ; 45 ; . Because of the broad range of neurologic and behavioral manifestation and varying severity, therapy of TS must be individualized and tailored specifically to the needs of the patient and the most troublesome symptoms should be targeted first 46 ; Table 1 ; . Treatment of Tics The goal of treatment should not be to completely eliminate all the tics, but to relieve ticrelated discomfort or embarrassment and to achieve a control of tics that allows the patient to function as normally as possible. Although controlled, double-blind, placebo-controlled trials, are lacking the dopamine receptor blocking drugs neuroleptics ; are considered the most effective anti-tic agents Table 1 ; . Haloperidol and pimozide are the only neuroleptics currently approved by the Food and Drug Administration for the treatment of TS. In one randomized, double-blind, controlled study pimozide was found to be superior to haloperidol with respect to efficacy and side effects 47 ; . Some clinicians, however, prefer risperidone 48, 49 ; , fluphenazine, thioridazine, trifluoperazine, molindone, thiothixene, and tiapride. It is not clear whether the atypical neuroleptics, such as clozapine, olanzapine, or quetiapine will be effective in the treatment of tics and other manifestations of TS, but ziprasidone was found in one study to decrease tic severity by 35% 50 ; . Tetrabenazine, a monoamine depleting and dopamine receptor blocking drug not yet available in the US ; , is a powerful anti-tic drug, and it has the advantage over the conventional neuroleptics in that it does not cause tardive dyskinesias 51 ; . Besides tardive dyskinesia, other side effects possibly associated with neuroleptics include sedation, depression, weight gain, school phobia, and hepatotoxicity. In addition, pimozide may prolong the QT interval and, therefore, it is recommended that an electrocardiogram is performed before starting pimozide, and that electrocardiogram is repeated three months later and at least once a year thereafter. Other drugs found to be useful in the treatment of tics include clonazepam, pergolide, cannabinoids, nicotine gum and transdermal nicotine patch, but none of these drugs have been studied by well-designed, placebo-controlled trials 52 ; . Focal motor and vocal tics have been also successfully treated with botulinum toxin injections in the affected muscles 53-55 ; . Such local chemodenervation ameliorates not only the involuntary movements but may also eliminate the premonitory sensory component. The benefits last on the average 3.
