Family Interview with Marla and Valerie Valerie and Marla's family consists of their parents and two sisters. Their older sister Carmen lives in Edmonton. Valerie lives by herself in Calgary. Marla and her youngest sister Sharon live at home with their parents Lena and Bob. Their 17 year old sister has had problems with her spine resulting in lots of pain. Sharon had back surgery and many complications postoperatively. Valerie and Marla spoke about Sharon's health care experience. Salient Themes: V Support 5. Recreational support a. age appropriate activities Learning Elements: Determining ways to better provide recreational support for teenage patients "I believe the child life worker came twice in all the time Sharon was in the hospital. Once they told us that bingo was starting and the other time she brought in some videos. I know they do more for younger kids. They do not seem to be that focused on teenagers. I think her mood would have been happier if she had someone to do things with. It would have given her less time to dwell on the fact that her arm and back hurt." "Part of the problem was that many of the activities were not age appropriate. We did attend bingo but the prizes were geared to 10 years and under. It would be great to have prizes and activities appropriate for teenagers. Sharon would have enjoyed doing activities with a volunteer. When we would visit her, we tried to get her to play games with us. We would bring those types of activities when we visited." "The teen room is there but if you go, there is no one in there. They need someone to encourage the teens to go to that room. They say you can use it but the room is always locked. You then need to find someone with the key to unlock it. That is an added step and if you have taken the effort to get there in the first place, you don't want to have to go searching for that person. You just sort of give up and go back to the room." Health Provider Discussion Questions: 1. In your health care role, what do you need to know in order to provide support to your teenage patient? 2. What suggestions do you have to enhance recreational support for the teenage patients in your health care facility? Parent Discussion Questions: 1. Describe the support your teenager received during his or her health care experience. 2. What can be done to improve the hospital experience for teens?.

Male Partners With Unsatisfactory Findings on Semen Analysis Although the period of abstinence necessary to obtain an optimal semen specimen has been the subject of debate, 2 to 4 days is usually recommended.18 An abnormal initial sperm count necessitates that more than one sample be analyzed to determine a trend. If there has been a history of febrile illness or major physiologic or psychologic stress within the previous 3 months, one full cycle of spermatogenesis approximately 72 days ; should be allowed before repeating the analysis.18 To diagnose male factor infertility, the semen analysis should be standardized and performed according to the guidelines of the World Health Organization.18 Unfortunately, the predictive value of the semen analysis is limited, even when properly performed, and reliable tests for sperm function do not yet exist.19 Most andrologic disorders are treated empirically.20 Based on treatment approach, male factor infertility can be divided into the following six categories: genetic causes, gonadotropin deficiency, anatomic defects, infections, immunologic causes, and idiopathic causes.

Immunological assessment Specific IgE or IgG antibodies against an occupational sensitizer have been detected mainly for high-molecular-weight agents. The value of in vivo or in vitro tests in establishing a sensitization to low-molecular-weight chemicals has proved limited. In addition, the presence of immediate skin reactivity or increased specific IgE or IgG may reflect exposure or sensitization but it does not imply that the target organ is involved. With high-molecularweight allergens, negative skin tests to such allergens almost completely exclude the possibility of OA. With low-molecular-weight allergens, such as isocyanates and red cedar, negative skin tests or specific IgE or IgG do not refute or confirm the diagnosis of OA; skin tests are also usually unavailable. Specific antibodies to allergens may be demonstrated in biological fluids using a variety of tests. They confirm a sensitization demonstrated by skin test but are often less sensitive. They represent an alternative to the skin test when the preparations of antigens have irritant, toxic, or mutagenic effects, and in patients under pharmacological treatment that blunts normal skin reactivity. Control for specificity is required, especially when protein conjugates are used. Different factors, such as total IgE level, characteristics of the conjugate, carrier specificity, and cross-reactivity with other antigens, may affect the results. Assessment of the chemokine monocyte chemoattractant protein-1 MCP-1 ; produced in vitro by diisocyanate-stimulated blood mononuclear cells exhibited higher test efficiency than specific antibodies for identification of isocyanate asthma. Other in vitro tests, such as histamine release from basophils, are less standardized but may be useful occasionally. Physiological assessment The presence of airway obstruction with demonstrable reversibility after inhaling a bronchodilator is a well-recognized confirmatory step for asthma. If there is no significant airway obstruction, the demonstration of increased bronchial responsiveness is suggestive of asthma, not necessarily of OA. Serial PEF monitoring has been proposed for both the investigation and assessment of asthma. The sensitivity and specificity of PEF monitoring, as compared with the `gold standard' specific inhalation challenges, varies from 72% to 89% depending on the study. Combining PEF and the assessment of eosinophils in induced sputum for periods at work and away from work may improve the diagnostic yield. Laboratory challenges in small cubicles were proposed by Pepys in the 1970s in an effort to reproduce the workplace environment. Improvement in the methodology of the test has been put forward using closed-circuit apparatus for dry particles and vapors, including isocyanates. After exposure, various temporal patterns of reactions can occur, including those of typical immediate, late, and dual ; and atypical reactions progressive, square waved, and prolonged immediate ; . Outcome The outcome of OA after diagnosis is often poor. Removal from the occupational exposure is associated with recovery from asthma in about 50% of subjects. Many retrospective studies have unanimously demonstrated a persistence of asthmatic symptoms, bronchial obstruction, and hyperresponsiveness in subjects with OA after being removed from exposure. Most studies also showed that the total duration of exposure, the duration of exposure after the onset of symptoms, and the severity of the asthma at the time of diagnosis are all determinants of the prognosis. Improvement in bronchial responsiveness occurs predominantly in the first two years after cessation of exposure and continues, though at a slower rate, later on. If exposure continues, there is overall deterioration in the asthmatic condition. Mapp CE, Boschetto P, Maestrelli P, Fabbri LM. J Respir Crit Care Med 2005; 172: 280-305.
